I have the exact same phone and same issue. I don't think it's just you. Unfortunately I don't have a sure fire solution but when iTunes Radio won't play I'll simply disconnect the phone, force close the music app, then restart. Usually this gets it to play correctly. Artists and song titles still remain intermittent though.
