I'd say it's also important to consider your mechanical knowledge and abilities as well. If you are someone who can do small fixes on your car and keep up the regular maintenance at home it probably isn't worth it. Catastrophic failure is very rare on properly maintained cars.
My dealer offered me 40k/4yr for around $4200. I turned it down because I know I could do most of the wear and tear items like suspension and control arms myself so it really didn't make financial sense to me.
That being said it's also about piece of mind. If you have the money and don't want the worry than spend it. It's just money, we can't take it with us when we go lol.
