Door lock not flush with trim

This happened to my passenger door a while ago, and now just happened on my drivers side door after having the panel removed to replace my side mirror. The lock button is recessed below the panel. I can't see anything wrong with the panel itself. Did I bend the metal clips somehow across the top of the door panel? Is the lock actuator rod bent?


It's supposed to look like this:
