Transferring fonts?

I bought a new computer and I want to transfer all the custom fonts I had on the previous computer, is there anyway to do this? Or at least differentiate the OS fonts from the ones I have installed so that I can re-download.
Both machines are windows 7

"Go to the source PC, open control panel, Fonts, select and copy to a flash drive or something similar then insert the flash drive into the target PC, open control panel,Fonts, click on File, Install new fonts and browse to your Flash drive and go. Reboot and the fonts should be there on the target PC."
